I hereby release Ilana Visotsky Art Studio, its staff, and representatives from any and all claims of liability or damages that may arise from my child’s participation in activities at the studio. Medical Consent: In the event of a minor injury, I authorize the staff to administer basic first aid. In the case of an emergency, I understand that efforts will be made to contact me immediately. Photo & Video Release: I understand that Ilana Visotsky Art Studio may occasionally take photos or videos during classes, events, and birthday parties to share on social media, our website, or for promotional purposes. If I do not wish for my child’s image to be used in this way, I will notify the studio in writing or in person, and the studio will respect and honor that request.
